4-H Record Books

PLEASE NOTE: The record book pages have been revised from previous years. Please use the NEW pages below in place of older versions you may have. These pages are now fillable PDF versions that can either be saved to your computer and completed electronically and then printed or printed to complete by hand (with your neatest, most legible handwriting of course!).

Portage County 4-H Record Books

Members are encouraged to complete a 4-H record book that highlights the 4-H member’s experiences and activities during the 4-H year. 4-H members earn recognition and awards by completing a record book. 

General Pages (For All Record Books)

4-H Record Books will help you keep track of the achievements and activities that you participate in throughout your 4-H career.  The information on the project record sheets may be used to fill out applications for awards, trips, scholarships, and resumes.  The information will help you put into writing your accomplishments, recording your growth and development in 4-H.

FYI: Not all projects have a custom page- if you cannot find a page for your specific project, please use the ‘General Project Page’. (We will be continuing to simplify & update the record book process and align these project pages with the new 2025-2026 Wisconsin 4-H Project Areas in future years!)

Record Books F.A.Q.’s…

Who Should Complete a Record Book?

While not required, we encourage all 4-H members to complete a record book or add to their record books each year. 4-H records should be filled out completely and accurately. Parents may oversee the child’s work, but a child must do their own writing. Encourage your 4-H member to work on the record book throughout the year. 4-H record books will be due to your local 4-H club in September. Names of younger members (grade 3-8) are submitted for county awards and older member record books to be judged on a county level are due to the Extension Office in mid-October. The books will be evaluated and then returned to your child.

Why Complete A Record Book?
